Here's how to pick the right one Two years ago, a patient walked into the Seven Bungalows clinic of orthopaedic physiotherapist Harshada Rajadhyaksha after suffering a slip disc while doing yoga. "The man, who was in his early 30s was a gym trainer. Though strong, he was not flexible. This particular asana required him to touch his toes while standing and his instructor tried to push him into the pose. A niggling pain in the lower back escalated into a sharp snap that led to a slip disc and hospitalisation for more than a month," she says. In her 25-year practice, Rajadhyaksha has noted that 99 per cent of fitness enthusiasts report some injury or the other at least once. The right trainer and dietician, then, is all that stands between you and a debilitating injury. Look for certification While fitness trainers in India are not regulated by any central body, there are several top gyms that run three-month programmes. Experts suggest looking at your trainer's course. Coursework, which typically equips a graduate to become a floor trainer, includes studying human anatomy and planning exercise programmes that suit clients. A good trainer will have a few years of experience with different body types to back him. Picking a certified dietician is equally important. Niti Desai, Consulting Dietician at Cumballa Hill Hospital, says dieticians undergo clinical training which helps them understand the various ways and combinations in which foods affect health. "In India, however, all certified nutritionists undergo the clinical training as well. Ensure that the one you consult has a diploma or a degree in dietetics or nutrition and not just a three-weekend course. They should have undergone the State exam to become a Registered Dietician," she observes. Are you being heard? Does your trainer understand your fitness goals and limitations? Vinod Chenna, a professional fitness trainer with 20 years of experience, says the best trainers are those who understand not only the client's body but also their lifestyles. "If you work in an IT company, then your trainer needs to understand that your lower back and neck are weak. Your trainer needs to then strengthen your weak muscles before proceeding to achieve your target — whether it's sculpting six packs or weight loss. Good trainers take into account age, stress levels, medical history and nutrition supply." Make sure that your trainer asks you these questions before lining up a routine. Chenna says, keeping with international standards, he goes that extra mile to be a reliable trainer. "The best way to understand training is to work on your self. Twice a year, I use different workout regimens to gain 30 kg and lose that weight to get back to my original six pack. So, I have practised weight loss and gain on my own body at least 40 times in the course of my career. If I understand what works for me, I will be able to understand my clients," he says. Your dietician, says nutritionist and fitness expert at Integym, Madhuri Ruia, should also take into account your workouts, your doctor's prescriptions and your daily meals. "A dependable nutritionist is a keen listener to not just your diet record but also your lifestyle. Are you a singleton or living with your in-laws? They understand your home scenario and can suggest suitable alterations to your diet. They also come up with customised diet charts." Don't buy the celeb card Most of us head straight to the trainer who has helped the current Bollywood favourite sculpt his surfboard abs. Althea Shah warns against falling into this trap. Shah, a fitness expert with Gold's Gym Fitness Institute, says, "You may like to choose a trainer based on reputation or style of working, but the best thing to do is to find one who will work with your body type." Shah suggests attending a few trial sessions to check if your trainer is giving you scientific explanations for the regimen. Do an upper body and a lower body exercise and see if the muscles are being worked, she adds. Starvation point? Look for warning signs when your nutritionist advises you to go on a crash diet or prescribes single food nutrition (such as cabbage for a week). A starvation plan can leave you with lowered immunity, excessive hairfall and muscle breakdown. A year ago, Desai counselled a 27-year-old who was to undergo a fibroid extraction and was found to have ketones in her blood, a primary symptom of diabetes.     "Ayushmanbhava" is a common blessing made by Indian elders and means "may you live a long life". Little do they know that running can add up to 3 years to your life! Dr. K. H. Srinivasa, Professor of Cardiology, Sri Jayadeva Institute of Cardiovascular Sciences and Research, Bangalore provides proof for the same via a study which examined the running habits of more than 55,000 adults with an average age of 44 years. Running routines of these individuals were assessed on a medical history questionnaire under leisure-time activity. They were followed for a period of 15 years during which time 3,413 deaths due to all causes and 1,217 deaths due to heart and blood vessel (jointly called "cardiovascular") disease occurred. Compared with nonrunners, runners had 30 per cent and 45% lower risks of all-cause and cardiovascular death, respectively, with 3 years added to their lifespan. Surprisingly, weekly running for less than 51 min, less than 9.6 km, 1 to 2 times or even at less than 9.6 km/h was sufficient to reduce risk of death, compared with not running. Though persistent runners reaped maximum benefits, people who ran for even 5 to 10 minutes/day at very slow speeds (less than 9.6 km/h) also gained significantly in terms of risk of death due to all-cause and cardiovascular causes. This health benefit remained irrespective of the individuals sex, age, body mass index, health behaviour and medical conditions. (Lee D et al. Dr. Bhavesh Vajifdar, Consultant and Interventional Cardiologist at Lilavati Hospital and Holy Family Hospital, Mumbai explains "Lack of activity destroys the good condition of every human being, while movement and methodical physical exercise can save it and preserve it.—Plato 400 B.C'. In India, up to 80% of individuals do not meet the exercise guidelines; which recommend 30 min/day of moderate-intensity exercise or 75 min/week of vigorous-intensity exercise. Lack of time is a major deterrant for exercise. Hence, establishing goals that can be attained with minimal effort is important. This new study by Dr. Duck-Chul Lee, shows that higher-intensity exercise yields more favourable effects on mortality and disease risk than lower-intensity exercise. The benefits of running are superior to walking. Running beats walking by a factor of 2:1 to 4:1. A 5-min run is as good as 15-min walk. For younger individuals who are pressed for time, running is a far better option for time efficiency. There are innumerable studies to implicate that the LSM not only take care of the disorders, but also reduce morbidity and mortality.   5. Researchers have found that women who eat a Mediterranean diet can keep age-related diseases at bay and live longer than others. The Mediterranean diet is rich in fruits, vegetables, fish, legumes and heart-healthy fats and greater adherence to such a diet is associated with longer telomeres, the biomarkers of ageing, the findings showed. "To our knowledge this is the largest population-based study specifically addressing the association between Mediterranean diet adherence and telomere length in healthy, middle-aged women," said Immaculata De Vivo, an associate professor at the Brigham and Women's Hospital (BWH) in the US. Telomeres are repetitive DNA sequences at the ends of chromosomes that get shorter every time a cell divides. Shorter telomeres have been associated with decreased life-expectancy and increased risk of ageing-related disease, while longer telomeres have been linked with longevity. "Our results further support the benefits of adherence to this diet to promote health and longevity," De Vivo added. The study involved 4,676 disease-free women. The researchers found that a greater adherence to the Mediterranean diet, and even small changes in diet, were associated with longer telomeres.   9. Regular, moderate consumption of filtered, decaffeinated coffee may decrease an individual's risk of developing Type 2 diabetes, promising research indicates. Filtered coffee exhibited a greater protective effect than boiled coffee and decaffeinated coffee showed a greater protective effect than caffeinated coffee. "Drinking decaffeinated filtered coffee at lunch time is the best time of day to lower the chances of diabetes," researchers noted. "The evidence shows that drinking three to four cups of coffee per day is associated with an approximate 25 percent lower risk of developing Type 2 diabetes compared to consuming none to less than two cups per day," stated researchers from Switzerland-based Institute for Scientific Information on Coffee (ISIC). They also suggested a favourable association: With each additional cup of coffee reducing the relative risk of developing Type 2 diabetes by 7-8 percent. The ISIC detailed the findings in its annual diabetes report outlining the latest research on coffee and Type 2 diabetes. According to a recent study, this may be a new sleep disorder A new study has found that sleep drunkenness disorder affects one in every seven people and causes episodes of waking up confused. Reports say that this disorder involves confusion or inappropriate behaviour such as answering the phone instead of turning off the alarm, during or following arousal from sleep, either during the first part of the night or in the morning. An episode of sleep drunkenness, say study authors, is often triggered by a forced awakening, and may even cause violent behaviour during sleep or amnesia of the episode. Study author Maurice M Ohayon, from Stanford University's School of Medicine, says, "These episodes of waking up confused have received considerably less attention than sleepwalking, even though the consequences can be just as serious." Our maximum city, sadly, has the maximum occurrence of sleep disorders, thanks to the hectic work life and the generally demanding lifestyle. The extreme travel conditions (people travel for four hours to and fro to office) plus the nine hours of work time leave very little time for sleep, and this chronic sleep depravation leads to many kinds of problems, not just at the physical level, but the emotional level as well. "Sleep, of good quantity and quality, is absolutely necessary for physical, mental and emotional well-being," says Dr Ramanathan Iyer, sleep consultant, adding, "Poor quality of sleep leads to a lot of issues." Psychiatrist Dr Harish Shetty states, "Sleep has a certain architecture, it has to follow some cycles of REM sleep, deep sleep, etc, and if that is not followed, a person could suffer from sleep drunkenness. When the quality of sleep gets is affected, and the sleep architecture is disturbed, then one becomes confused." Sleep inertia, says Dr Iyer, is different from sleep drunkenness. A characteristic of sleep inertia involves the feeling of wanting to go back to sleep as soon as you wake up to your alarm. It is about that typical groggy feeling when, with half-closed eyes, you also have impaired alertness and there is a significant decline in your motor dexterity. When one is awakened abruptly, one's mental and even physical performance is not up to the mark, however adequately the person may function, explains Dr Iyer. However, in sleep drunkenness, this may not be the case always. About The research and its findings Researchers from Stanford University's School of Medicine asked 19,136 people aged 18 and above about their sleep habits and whether they had experienced any symptoms of the disorder, and about mental illness diagnoses and any medications they took. The findings showed that 15% of the group had experienced an episode in the last year, with more than half reporting more than one episode per week. In the majority of cases — about 84% — people with sleep drunkenness also had a sleep disorder, a mental health disorder or were taking psychotropic drugs such as anti-depressants. Less than 1% of the people with sleep drunkenness had no known cause or related condition. Both long and short sleep times were associated with the sleep disorder. About 20% of those getting less than six hours of sleep per night and 15% of those getting at least nine hours experienced sleep drunkenness. Consumption of one 28g serving of yogurt per day is associated with an 18 percent lower risk of Type 2 diabetes, the findings showed. "We found that higher intake of yogurt is associated with a reduced risk of Type 2 diabetes, whereas other dairy foods and consumption of total dairy did not show this association," said senior researcher of the study Frank Hu from the Harvard School of Public Health in the US. The findings suggest that yogurt can be incorporated into a healthy dietary pattern, Hu added. Type 2 diabetes is a chronic condition that occurs when the body does not produce enough insulin, or the body's cells develop resistance to insulin. It has been shown that probiotic bacteria found in yoghurt improve fat profiles and antioxidant status in people with Type 2 diabetes and the researchers suggest this could have a risk-lowering effect in developing the condition. The researchers pooled the results of three prospective cohort studies that followed the medical history and lifestyle habits of health professionals. The study involved a total of 289,900 participants. Within the three cohorts, 15,156 cases of Type 2 diabetes were identified during the follow-up period. When adjusting for chronic disease risk factors such as age and BMI (body mass index) as well as dietary factors, it was found that high consumption of yogurt was associated with a lower risk of developing Type 2 diabetes. The findings were confirmed when the authors conducted a meta-analysis, incorporating their results and other published studies up to March 2013. The study appeared in the journal BMC Medicine.
